New clients:
Please make sure you fill out the intake form online when booking. Your first visit will include a full health history, assessments, discussion of treatment plan, treatment, and home care recommendations.

Please if you feeling unwell cancel/reschedule your appointment as soon as you are aware that you may not be able to attend.
Cancellation
**If you must cancel or rebook your appointment it is respectfully requested that you provide at least 24 hours notice. Cancellation without 12 hours notice will result in a %50 fee. Missed appointments could result in %100 fee.
